Michelle P. Waiver (MPW) Services
The Michelle P. Waiver is a Kentucky Medicaid 1915(c) Home and Community Based Waiver Service designed for individuals with developmental or intellectual disabilities to help them live in their communities as independently as possible. The MPW services provided at our agency include:

Targeted Case Management (TCM)
Targeted case management is a structured service that provides personalized support to help individuals with specific needs gain access to needed medical, mental health, substance abuse, social, educational, housing, vocational and other community services.

- Assessment
- Care planning
- Referral and linkage
- Coordination
- Monitoring
*The overall goal of case management is to improve functioning, promote stability and bridge gaps in services.
